Job Title: Primary Care Diabetes Advisor

Chemistree Solutions Limited is seeking a highly skilled and dedicated Nurse Adviser to join our team. This is an exciting opportunity to make a real difference in the lives of patients with Diabetes.

As a Nurse Adviser, you will play a vital role in delivering high-quality care to patients with Diabetes in line with National guidelines. You will work closely with the NHS in primary care to develop and implement a nursing programme that drives best practice and improves patient outcomes.

This is a challenging yet rewarding role that requires strong clinical skills, excellent communication skills, and the ability to work collaboratively as part of a team.

About the Role:

We are looking for an experienced Nurse Adviser who has a strong background in managing Diabetes in primary care. The successful candidate will have a current NMC registration, a Diabetes Diploma or equivalent, and a full UK driving licence.

Key Responsibilities:
  • Deliver high-quality care to patients with Diabetes in line with National guidelines.
  • Work with the NHS in primary care to develop and implement a nursing programme that drives best practice and improves patient outcomes.
  • Collaborate with healthcare professionals and patients to make informed decisions about treatment pathways in Diabetes.

Requirements:

The ideal candidate will have:
  • 3+ years experience managing Diabetes in primary care.
  • Knowledgeable and confident in providing education, support, and advice to people with Type 2 Diabetes.
  • Skills and knowledge in optimising oral hypoglycaemics.
  • IT literacy, including primary care systems.

Compensation: We offer a competitive basic salary of £55,000 per annum, plus a range of benefits including company car or car allowance, bonus, NMC/RCN fees, pension scheme, private health, and additional package benefits.
