Nurse Practitioner in Diabetes Management

Preston Park Community Primary Care Network is seeking a highly skilled and experienced Nurse Practitioner to join our team. As a key member of our multidisciplinary team, you will provide specialist care and support to patients with diabetes, leading to improved patient outcomes.

The ideal candidate will possess a strong understanding of diabetes pathophysiology, treatment options, and lifestyle interventions. With extensive experience in managing complex cases, including insulin initiating and adjustment, you will be able to develop and implement individualised care plans to optimise blood sugar control, reduce complications, and improve quality of life.

As a Nurse Practitioner, you will work closely with GPs, pharmacists, dietitians, and other healthcare professionals to deliver coordinated care for diabetic patients. Your excellent communication skills will enable you to educate and support patients, families, and other healthcare professionals effectively.

Key Responsibilities:

  • Lead in the management of patients with Type 1, Type 2, and complex diabetes, including those requiring insulin therapy or complex regimes.
  • Develop and implement individualised care plans to optimise blood sugar control, reduce complications, and improve quality of life.
  • Educate patients and their families/carers on diabetes management, including diet, exercise, medication adherence, and monitoring.

Requirements:

  • Registered Nurse with valid NMC registration
  • Postgraduate certification or diploma at Level 7 in Diabetes Care
  • Extensive experience in managing diabetes care, including complex cases such as insulin initiating and adjustment.

Estimated Salary Range: £40,000.00-£42,000.00 per year
