Job Description

Barts Health NHS Trust is seeking an exceptional individual to fill the role of Lead Community Diabetes Specialist Nurse. This position offers a unique opportunity to make a significant impact on the lives of patients living with diabetes in Tower Hamlet Borough.

The successful candidate will be responsible for leading a team of nursing staff, providing specialist advice and support to patients, and working closely with GP practices and other healthcare professionals to ensure seamless coordination of care.

About the Role
  • The post holder will be responsible for developing and implementing effective diabetes care plans, working closely with the multidisciplinary team to ensure high-quality patient care.
  • You will provide ongoing professional development and mentorship to nursing staff, promoting excellence in diabetes nursing practice and ensuring consistently high standards of care.
  • As a clinical leader, you will contribute to the enhancement of diabetes nursing practice, sharing your knowledge and expertise with colleagues and supporting the development of new initiatives.
Requirements
  • Significant experience as a Band 7 DSN in the community, primary care, and/or inpatient setting.
  • Experience of change management, service improvement, and service planning and development.
  • Participation in nursing research and audit.
  • Leadership and management qualifications, such as a Master's degree in Leadership or Management.
Salary and Benefits

The salary for this role is £60,000 - £72,000 per annum, depending on experience. In addition to a competitive salary, we offer a range of benefits, including:
