The successful candidate will be responsible for leading the development and implementation of specialist nursing services in primary care, supporting the Northwest London Diabetes Transformation Programme.
This role involves identifying opportunities for service growth and redesign of community-based diabetes care, including implementing care pathways and developing partnerships with primary and secondary care to ensure integrated care across West London Borough.
The aim is to enhance and transform diabetes care for individuals in North West and West London boroughs.
Key Responsibilities:
- Lead the development and implementation of specialist nursing services in primary care.
- Support the Northwest London Diabetes Transformation Programme.
- Identify opportunities for service growth and redesign of community-based diabetes care.
- Implement care pathways and develop partnerships with primary and secondary care.
- Provide clinical leadership and vision to support Diabetes Specialist Nurses.
