Main duties of the job

Thesuccessful candidate will manage a caseload of around 25 people, they will be responsiblefor arranging care, liaison with other professionals and agencies, manage andreview care plans and risk assessments ensuring these are done collaborativelyin a recovery focused manner. They will also ensure CPA reviews are completedin a timely manner and to a high standard and are inclusive, keeping the personand their carers in the centre of all we do.

The team operate using the flexibleassertive community treatment model and we use a shift coordinator system on arota. This role requires the practitioner to delegate and prioritise work,oversee and facilitate the daily morning meetings, managing the team diary andcarry out any urgent visits.

The successful candidate will be anefficient organiser, managing their workload and be flexible to meet the needsof service. They will be supported to manage their own requirements forsupervision in line with their professional registration.
