Being a specialist secondary care service, we provideevidence-based interventions to service users with varied and complex mentalhealth needs alongside age related and physical health needs.
The successful candidate will work within the MDT andindependently, provide a holistic, comprehensive and high quality service thatis collaborative and recovery focused. They will have excellent clinicalskills, effective communication skillsand experience working with severe and enduring mental illness. The successfulapplicant will participate and contribute in the assessment andformulation, planning and implementationof evidence-based care for older people. This will involve medicationadministration, physical health monitoring, and co-facilitating groups.

Main duties of the job
As a NMC registered Mental Health Nurse you will ;
*Work within your competence, be accountable and responsiblefor your actions, in line with the NMC.
*Work independently and within the MDT, under the guidanceof senior practitioners.
*Collaboratively and comprehensively assess the needs ofservice users, plan, implement and reviewcare.
*Identify, support and manage risk with service users andtheir families. including recognising and reporting safeguarding.
*Undertake physical health monitoring.
*Support delivery of psychosocial interventions.
*Support monitoring of prescribed medication.
*Liaise with families and carers and other professionals.
