We are seeking a skilled and compassionate Community Mental Health Nurse to join our team at South West Yorkshire Partnership NHS Foundation Trust.
Job Summary:
The successful candidate will work in a supportive environment alongside experienced staff, providing initial assessments for service users referred to secondary care mental health teams.
Key Responsibilities:
- Conduct initial assessments in various settings for service users referred to secondary care mental health services.
- Develop and implement care plans in partnership with service users and carers.
- Provide supervision and support to other team members.
- Assess and manage risk in complex mental health situations.
- Collaborate with other professionals to promote health and well-being.
Requirements:
- Significant experience in mental health settings.
- Ability to assess and manage risk in various settings.
- Experience in supporting and mentoring colleagues.
- Relevant post-registration qualifications and training.
