Community Health Nurse Specialist

We are seeking a skilled and compassionate Community Health Nurse to join our team at South West Yorkshire Partnership NHS Foundation Trust. As a Community Health Nurse, you will play a vital role in delivering high-quality care to patients in their own homes, promoting independence, and improving health outcomes.
Job Summary:The successful candidate will be responsible for:
- Providing evidence-based planned care to patients in their own homes, ensuring safe and effective care that meets individual needs.
- Working collaboratively with the District Nurse and other healthcare professionals to develop and implement care plans that promote patient independence and well-being.
- Conducting assessments, planning, implementing, and evaluating care for patients, reporting and escalating relevant information to appropriate healthcare professionals.
- Participating in health screening programs and disease management of patients under the direction of the District Nurse.
- Establishing and maintaining excellent relationships with members of the Primary Health Care Team and other healthcare professionals, initiating referrals where appropriate to ensure adequate support for patients and carers.
The successful candidate will be responsible for:
- Delivering high-quality, person-centred care to patients in the community, ensuring regular evaluation of care and developing systems to support staff interventions and care quality.
- Working collaboratively with individuals, formal and informal carers, and other services to promote the concept of self-care and patient-led care, providing appropriate education and support to maximise patient independence and understanding of their condition(s).
- Leading and fostering a culture of openness and recognition of duty of candour, inspiring a shared purpose to support the delivery of high-quality effective care.
- Contributing to the development, collation, monitoring, and evaluation of data relating to service improvement and development, quality assurance, quality improvement, and governance, reporting incidents and developments related to community nursing.
To be successful in this role, you will need:
- A Registered Adult Nurse qualification.
- Evidence of health promotion skills and health education/promotion skills.
- Understanding of government legislation/documentation affecting NHS and primary care.
- Able to fulfil Occupational Health requirements for the post (with reasonable adjustments if necessary).
- A current driving license and access to a car during the working day (reasonable adjustments will be considered for any applicants who are unable to drive due to a disability).
