Job summary

We are looking to recruit a Clinical Pharmacist to work within North Birmingham Primary Care Network. The PCN consists of 6 practices working collaboratively to look after over 40,000 patients.

The successful candidate will have a post-graduate diploma or higher degree in clinical pharmacy and will be, or working towards being, an independent prescriber.

The successful candidate will be expected to provide a leadership role on quality and improvement, clinical audits as well as managing some aspect of the Quality and Outcome Framework (QOF).

Main duties of the job

The successful candidate will perform remote and face-to-face medication reviews of patients, especially for older people, people resident in care and nursing homes and those with multiple co-morbidities and polypharmacy.

They will have a role in managing long-term conditions, working directly with patients to assess and treat conditions within their professional competencies as well as promote self-care.

They will work as part of a multi-disciplinary team (MDT) to develop and run processes for repeat prescription reauthorisation, management of medicines on transfer of care from secondary, intermediate and other primary care organisations.

They will be part of the team for safe prescribing.

Job responsibilities

Thepost holder is a pharmacist, who acts within their professional boundaries,supporting and working alongside a team of pharmacists in general practice. Thepost holder will work as part of a multi-disciplinary team in a patient-facingrole. The post holder will take responsibility for areas of chronic diseasemanagement within the practice and undertake clinical medication reviews toproactively manage patients with complex polypharmacy.

Thepost holder will provide primary support to general practice staff with regardsto prescription and medication queries. They will help support the repeatprescription system, deal with acute prescription requests, and medicinesreconciliation on transfer of care and systems for safer prescribing, providingexpertise in clinical medicines advice while addressing both public and socialcare needs of patient in the GP practices of NBPCN.

The post holder willbe responsible to hold an appropriate indemnity cover for their clinical work.

Thepost holder will provide clinical leadership on medicines optimisation andquality improvement and manage some aspects of the quality and outcomesframework and enhanced services.

Thepost holder will ensure that the practice integrates with community andhospital pharmacy to help utilise skill mix, improve patient outcomes, ensurebetter access to healthcare and help manage workload. The role is pivotal toimproving the quality of care and operational efficiencies so requiresmotivation and passion to deliver excellent service within practices of NBPCN.

Thepost holder will be supported to develop their role to become a non-medicalprescriber, if they are not already a prescriber.
