Main duties of the job

Social prescribing empowers people to take control of their health andwellbeing. Non-medical link workers give time to individuals and focus onwhat matters to me they take an holistic approach, connecting people tocommunity groups and statutory services for practical and emotional support.Link workers support existing groups to be accessible and sustainable and helppeople to start new community groups, working collaboratively with all localpartners. The Social Prescriber post holder will be working in partnership with theexisting SPLW and the practice teams in their Primary Care Network (PCN) tosupport patients. The Social Prescriber will have a good understanding ofsocial prescribing, well-being and community services together with a goodunderstanding of and ability to provide motivational and behavioraltechniques.

About us

Northolt Primary Care Network started in the year 2019. We have 6 GP practices in the PCN. You will be working with 1 or more practices within the PCN.

You will be reporting to the PCN clinical Director and the PCN Board. Your Day to day clinical supervision is received from the Partner or their Nominated Deputy in the practice in which you are working or attach to.

We aim to Provide high quality accessible primary medical care. To follow up to date guidelines for best clinical practice. To ensure and support continued professional development of the Primary Health Care Team To provide an open, safe and pleasant environment for staff and patients alike To treat all patients in a polite, fair and equitable manner. We will respect the individuality of patients

Social prescribing can help to strengthen community resilience and personalresilience, and reduces health inequalities by addressing the widerdeterminants of health, such as debt, poor housing and physical inactivity, byincreasing peoples active involvement with their local communities. Itparticularly works for people with long-term conditions (including support formental health), for people who are lonely or isolated, or have complex socialneeds which affect their wellbeing.Car Driver is essential for this role as you will be travelling across the PCNgeography on a regular basis

Skills and Knowledge

Essential

Demonstrable experience of working directly in a community development context, adult health and social care, learning support, public support or public health/health improvement. Experience of working with the general public. Experience of working within the NHS/Social Care. Experience of working within a multi-disciplinary team. Experience of working under pressure and managing varying workloads and changing demands. Excellent communication skills with the ability to communicate orally and in written form in a confident, articulate manner. Ability to build relationships with others to enable and support collaborative working. Relevant experience of General Practice IT systems

Experience of supporting people, their families and carers in a related role (including unpaid work) Experience of partnership/collaborative working and of building relationships across a variety of organisations. Experience of data collection and providing monitoring information to assess the impact of services Full driving license and access to a car. Ability to travel across the locality on a regular basis, including to visit people in their own homes

Qualifications

Essential

.GCSE grade A* to C (9-4) in English and Maths or equivalent .Demonstrable commitment to professional and personal development .Willingness to work towards further educational qualifications required for the role
