We are seeking an enthusiastic and empathetic individual to join our team as a Social Prescribing Link Worker. As a key member of our Primary Care Network, you will play a vital role in supporting patients with their health and wellbeing needs.
Job Responsibilities:
As a Social Prescribing Link Worker, you will work closely with our practice teams to identify patients who would benefit from social prescribing. You will take an holistic approach, connecting people to community groups and statutory services for practical and emotional support. Your role will involve building relationships with patients, their families, and carers, and providing them with the support and guidance they need to improve their health and wellbeing.
Key Skills and Qualifications:
* Demonstrable experience of working directly in a community development context, adult health and social care, learning support, public support or public health/health improvement.
* Excellent communication skills with the ability to communicate orally and in written form in a confident, articulate manner.
* Ability to build relationships with others to enable and support collaborative working.
* Relevant experience of General Practice IT systems.
