The Programme Director - Integrated Pathways plays a key role in developing, improving, and delivering services across the ICS, working in conjunction with system partners. The primary purpose of the role is to ensure that the integrated pathways programme delivers on the purpose above.

Main duties of the job

This includes:

  • Working with residents, service providers, local borough-based partnerships, and primary care networks, to determine our future strategy and approach;
  • Developing a five-year plan for the programme;
  • Ensuring that all service provision is aligned and complementary, and
  • Pathways are effective, efficient, and equitable and that services are comprehensive and not duplicative.

The Integrated Pathways team work with NWL's borough teams and providers to develop, improve, and deliver care pathways that deliver as seamless an experience as possible to our residents. These care pathways span multiple providers and other partners, and include both Urgent and Emergency Care and Planned Care services.

Job responsibilities

Strategy, Planning, and Organisation

Developing Strategy and Delivering Transformation:

  • Be responsible for leading the production and delivery of the Integrated Pathways elements of the ICS's strategy, the five-year Joint Forward Plan, and the annual plan.
  • Working with borough teams and borough-based partnerships, lead the planning and design of the strategy to meet the vision and strategic direction.
  • Manage the activities necessary to ensure delivery of a transformational strategy.
  • Chair or attend as appropriate, meetings with varied internal and external key stakeholders to facilitate the delivery of the strategic objectives.
  • Responsibility for ensuring the function/strategy is delivered to time, to quality standards, and in a cost-effective manner, adjusting plans and resources as required.
  • Ensuring that the delivery of the function/strategy is to the appropriate levels of quality, on time, and within budget, in accordance with the programme plan.
  • Provide strategic leadership across organizational and sector boundaries and pathways to facilitate transformational change for the ICS population.
  • Ensure that the Integrated Pathways team influences and seeks input from the wider ICS system including borough teams, borough-based partnerships, provider collaboratives, public health, local government, local politicians, the voluntary and community sector, other partners, and local people to make real transformational change for residents.
  • Ensure that there are effective mechanisms for anticipating, identifying, and responding to key contracting, commissioning, and financial risks arising from contractual arrangements within the portfolio that could impact on the successful delivery of the ICB strategy.
  • Ensure appropriate stakeholder engagement strategy is in place defining how the strategy will engage with all stakeholder groups and what information flows will be established and maintained.
  • Support the delivery of the NHS's commitment to reduce carbon emissions by 80% by 2028 (emissions under NHS direct control), working with the responsible director to deliver interventions which reduce consumable waste and ensure energy-efficient estate.

Operational

Deliver effective programmes of work across the Integrated Pathways Portfolio:

  • Drive and facilitate collective responsibility for improving whole pathways and removing organizational and sector barriers to accessing health and care services through strategic commissioning, contracting, and population health approaches;
  • Oversee the work to ensure work streams are integrated with the overall strategy; ensuring synergy between strategy milestones and objectives;
  • Support borough teams and borough-based partnerships to implement the integrated pathway programme;
  • Negotiating with and influencing board-level system leaders and wider ICS partners will be a key responsibility of this role to ensure that progressive transformational change can be achieved.
  • Manage, monitor, and report on benefits realization management, tracking the progress and ensuring that the intended benefits are achieved with outcomes maximized.
  • Ensure appropriate governance structures are in place to support delivery and transformation.
  • Define and implement business processes that support the functions of the new ICS structure.
  • Manage the appropriate program controls ensuring monitoring and control activities and performance targets are on track.
  • Ensure there is a resource management plan which sets out the activities required to implement. (Resources meaning finances, people, assets, and technology).
  • Manage risks ensuring they are appropriately identified and controls and/or mitigation are in place, escalating to the ICB board as appropriate.

Driving high-quality outcomes for residents:

  • Work with the Medical and Nursing Directors, professional leads, and the ICB CMO to collectively oversee the quality of all services within the portfolio, including implementing a safer culture, safer systems, and safer care.
  • Be informed by the work of Clinical Reference Groups in order to commission high-quality health and care services.
  • Address underperformance in a timely manner and promote continuous quality improvement through learning, improvement methodologies, research, and innovation. This will include embedding delivery of cross-sector/organizational transformation and efficiency programs, related recovery programs, and actions that can support performance improvement.
  • Ensure that appropriate and effective contracting monitoring and performance arrangements are in place to ensure the delivery of effective primary and local care services.
  • Support executive colleagues in influencing strategic change and collaborative initiatives, such as collective financial and contracting opportunities, future workforce supply, quality and safety initiatives, and system-wide joint working at a strategic board level with system partners.
  • Promote awareness and understanding of contracting, value for money, and commercial issues, including robust and considered challenges.

Tackling health inequalities

  • Reducing health inequalities is a core objective of NWL ICS and the Director will promote and enhance strategic data-driven approaches to identifying and tackling health inequalities.
  • Work alongside the health equity program and with primary care, PCNs, Integrated Neighbourhood Teams, and Borough-Based Partnerships to understand local health needs and work with communities in innovative ways to improve health outcomes;
  • Be responsible for ensuring the delivery of the relevant elements of the health equity program (including the North West London's Core20Plus 5 program);
  • Support the ICB's commitment to address discrimination against NHS and staff from ethnic minority backgrounds, contributing to achieving the workforce race equality standard (WRES) and using their leadership to develop cultures of diversity and inclusion in the ICS.

Governance and Assurance

  • Support a strong culture of public accountability, probity, and governance, ensuring that appropriate and compliant structures, systems, and process are in place to minimize risk and promote the freedom to speak up.
  • Be accountable for ensuring that strategic commissioning and contracting activities are undertaken in compliance with standing financial instructions and standing orders and reporting arrangements for the ICB and wider partnership organizations across the system as necessary.
  • Where services are commissioned by teams hosted in other organizations work with the Chief Financial Officer to oversee successful performance on the use of resources in line with the agreed system oversight framework for the ICS, ensuring appropriate escalation arrangements are in place and appropriate actions are taken as necessary.
  • Lead on the identification of contractual performance risks within the portfolio and work with relevant providers and partners to enable solutions, including making recommendations for informal/formal intervention where appropriate.
  • Ensure that the ICB meets the required procurement and contractual governance arrangements as a statutory body within the portfolio
  • Work closely with the ICS clinical leadership to ensure appropriate clinical expertise is available to support pathway redesign, commissioning & contracting decision making
  • Take action to ensure underperformance in any service in the portfolio is addressed in a timely manner, working with the relevant providers as required.

Relationship Development and Management

  • Operate effectively in a flexible and demanding environment and proactively engage with stakeholders.
