Job overview

We are a diverse, multi-disciplinary team working across both North and South Derbyshire, supporting adults with autism who are at risk of placement breakdown or hospital admission. Our aim is to provide the right support and care to autistic individuals in the community to prevent hospital admissions.

As the Lead Nurse, you will be part of a fast-paced working environment, contributing to assessments, planning, and delivering bespoke person-centred care. You will represent the Specialist Autism Team (SAT) when emergency meetings are called to discuss plans for individuals who may be in crisis.

You will have experience supporting autistic individuals who have been inpatients at a mental health hospital, ensuring they receive appropriate advice and support to prevent further hospital admissions. You will be part of the multi-disciplinary team (MDT), coordinating and leading the Specialist Autism Team's pathway into the community.

You will be confident in forming the team's duty rota, taking the lead on duty calls as they come into the team. You will have knowledge of how community teams support autistic individuals, be able to refer to other services, and be involved in the initial assessment process for referrals coming into the service.

Main duties of the job

You will complete assessments individually, and support assessments completed by a clinician within the Specialist Autism Team and be actively involved in the delivery of direct patient care. You will provide individual and patient centred holistic care.

You will support the patient’s physical, psychological, emotional and autistic needs. You will have knowledge of safeguarding adults and have worked delivering care that promotes independence and well-being.

You will be an excellent communicator with adults with Autism who may be experiencing a crisis. You will be calm under pressure, work effectively alongside colleagues performing delegated responsibilities.

You will be expected to travel to both North and South Derbyshire sometimes at short notice, so a car is essential. You will provide support to people with Autism in the community at their place of residence, which could include residential placements & temporary accommodation.

You will work alongside the Hospital InReach team, Crisis Team, CMHT, Social Care colleagues and with our Short Term Intervention Team colleagues. You will form part of the joined up working essential to ensure safe and successful re-introduction back into community after a hospital admission.

The Lead Nurse role is an essential and very valued position. You will contribute, alongside your Lead Nurse colleagues, in the improvements in the care we deliver and the way it is delivered .

Detailed job description and main responsibilities

You have a key role in the development of the service and will contribute to team outcomes. Team development will run alongside your patient contact.

You will supervise less senior colleagues, ensuring delegated work is completed promptly. 

You will be calm under pressure and have a positive approach to problem solving. 

On-call may be a requirement in the future for this post

You will work as an effective member of SAT. You will organise own caseload, undertake specialist interventions with clients following assessment findings. You will have a good understanding of how to support Autistic people in situations that they find difficult and challenging and be an advocate for them. 

You will demonstrate a high level of clinical problem solving and be able to communite effectively with people with varied and complex needs, carrying out risk assessments and ensuring that information is cascaded to other team members.

You will be able to work independantly and manage your own diary, however, you will also be expected to contribute to the duty rota system alongside your colleagues. 

You will liaise and negotiate with other agencies, service users, carers and professionals when planning and evaluating care for individual clients whilst following data protection act and Caldicott principles.

You will ensure appropriate referrals to other agencies are made and chair and participate in a variety of meetings relevant to individuals, team developments and service developments.

You will model and teach packages of care/support on an individualised basis supporting and teaching other colleagues. You may also be asked to participate in the delivery of training to families, other agencies and at Autism events in the future. 

You will work alongside the Hospital InReach team, Crisis Team, CMHT, Social Care colleagues and with our Short Term Intervention Team colleagues in often high risk / highly stressful circumstances. You will have the skills to work effectively under such pressures and be able to act appropriately to support the autistic person, your colleagues and to manage your own well being. 

You will have knowledge of how mental health and Autism affect people, and support individuals who experience a range of mental health problems and challenges with their Autism. You will have good knowledge of how services work together and manage and coordinate your team to ensure that positive outcomes are achieved, some of these will be working directly with the autistic person post discharge from hospital if they have a date to leave pending.
