About the Role

We are seeking a highly skilled and dedicated Specialist Community Public Health Nurse to join our team at Derbyshire Healthcare NHS Foundation Trust. As a key member of our multi-disciplinary team, you will play a vital role in improving the health and wellbeing of children and young people in Derby City.

Key Responsibilities
  • Deliver high-quality public health services to children and young people, working closely with families, carers, and the wider educational system.
  • Assess health needs of children and young people and families, and develop effective plans to address these needs.
  • Lead a skilled mixed team, providing supervision and guidance to ensure the delivery of excellent patient care.
  • Develop and implement evidence-based practice, working in partnership with colleagues from statutory and voluntary agencies.
  • Work collaboratively with the management team to deliver the service specification, ensuring the highest standards of care and service delivery.
Requirements
  • Registered nurse with a SCHNP qualification.
  • Excellent communication and leadership skills, with the ability to manage own workload and prioritize tasks effectively.
  • Good understanding of public health roles and responsibilities, with a commitment to developing public health nursing practice.
  • Experience of working with corporate case loads and leading a skilled mixed team is desirable.
