Job Overview

We are seeking a skilled and compassionate Community Health Nurse to join our team at Derbyshire Healthcare NHS Foundation Trust. As a Community Health Nurse, you will play a vital role in delivering high-quality health visiting services to families and children in Derby City.

Main Responsibilities
  • Provide evidence-based public health nursing services to a defined corporate caseload, supporting and enabling families and children to make informed choices about their healthcare and promote health and wellbeing.
  • Lead on the delivery of the Healthy Child Programme, identifying packages of care based on the Universal, Universal plus, and Universal Partnership plus models of service delivery.
  • Work collaboratively with other agencies and partners to ensure a seamless delivery of services.
  • Supervise, monitor, mentor, and assume specific line management duties as required.
  • Communicate complex information regarding assessment and analysis of health needs of children and their families in routine and highly emotive situations.
Requirements
  • Registered nurse or soon-to-qualify.
  • Excellent communication skills, both verbal and written.
  • Experience of working as part of a team.
  • Basic IT skills.
  • Knowledge of 0- services, experience of Safeguarding, and understanding of national and local public health agendas.
