We are seeking a skilled and compassionate Community Health Nurse to join our team at Derbyshire Healthcare NHS Foundation Trust. As a key member of our Public Health Team, you will play a vital role in delivering high-quality health visiting services to families and children in Derby City.
Main Responsibilities- Provide evidence-based public health nursing services to a defined corporate caseload, supporting and enabling families and children to make informed choices about their healthcare and promote health and wellbeing.
- Lead on the delivery of the Healthy Child Programme, identifying packages of care based on the Universal, Universal plus, and Universal Partnership plus models of service delivery.
- Work collaboratively with other agencies and partners to ensure a seamless delivery of services.
- Supervise, monitor, mentor, and assume specific line management duties as required.
- Communicate complex information regarding assessment and analysis of health needs of children and their families in routine and highly emotive situations.
- Registered nurse or soon-to-qualify.
- Excellent communication skills, both verbal and written.
- Experience of working as part of a team and knowledge of 0- services.
- Understanding of national and local public health agendas.
