Job Summary

We are seeking a highly skilled and dedicated Registered Nurse to join our Rapid Response team in Derby City. As a Community Staff Nurse, you will play a vital role in providing high-quality nursing care to patients in their own homes, preventing hospital admission and supporting safe discharge.

About the Role

As a Community Staff Nurse, you will be responsible for assessing, planning, implementing, and evaluating individualized patient care in the most efficient and clinically effective way. You will work closely with our integrated community teams, GP surgeries, and local acute trusts to provide holistic care that enables patients to remain independent in their own homes.

Key Responsibilities
  • Provide high-quality nursing care to patients in their own homes
  • Assess, plan, implement, and evaluate individualized patient care
  • Work closely with integrated community teams, GP surgeries, and local acute trusts
  • Provide advice and support to junior staff
  • Implement referrals to relevant services as appropriate
Requirements
  • Full NMC registration as a Registered Nurse
  • Experience of working in the community
  • Knowledge of local and national priorities
  • Understanding of the wider NHS agenda regarding community services
