An exciting opportunity has arisen for an experiencedcardiology nurse to work within cardiology services across both South Tynesideand Sunderland Royal Hospital sites. This dynamic nurse will work within thewell-established Cardiology team leading the management and care delivery forpatients being admitted to our chest pain assessment unit at Sunderland RoyalHospital and accessing services at our South Tyneside hospital. The CNP rolewill consist of providing expert clinical knowledge to patients with cardiologyconditions. They will be a requirement to contribute to the ongoing developmentand delivery of nurse lead service for patients.

The post holder will hold well-developed interpersonal and communication skills and should also possess good facilitation and IT skills. You must be able to demonstrate the ability to work as an effective member of a multidisciplinary team, having excellent clinical skills to deliver exceptional standards of care for all patients and their families.

Main duties of the job

The post holderwill join the team in delivery of a 24/7 service with rotational day and nightduties. It is expected the post holder will have sound cardiology knowledge andskills in management of patients in the acute setting. Including thoserequiring PPCI and have unstable coronary disease.

We are also excited to offer this position as adevelopmental post for the right candidate who does not possess non-medicalprescribing but would be willing to work towards it. This would be as a traineeCNP in a band 6 role till training is completed.

The candidate must bewell organised and enthusiastic in participating with service developmentthrough audit and peer review. Helping to drive and sustain new initiatives tomeet local and national standards, as well as ensuring care meets our own Trustvalues.

To coordinate the delivery and development of the chest pain service through the use of effective evidence based practice in line with agreed national standards for patient safety and monitoring. To work closely with the Medical, Nursing and Professionals Allied to Medicine teams within the Trust to facilitate the prompt, evidence based treatment of patients with chest pain. To triage patients, prioritising clinical need and highlighting those patients who require immediate medical intervention to the cardiology/ medical team. To work autonomously, admitting and clerking appropriate emergency / chest pain admissions, assessing individual needs, undertaking and reporting on biomedical investigations, determining a differential diagnosis and initiating appropriate holistic, evidenced-based treatment and care. To make autonomous decisions regarding admitting or discharging patients within agreed protocols and arranging outpatient appointments for those being discharged under nurse-led protocols To support the Emergency Department with the assessment and management of patients with suspected cardiac chest pain according to the chest pain pathway by providing an in-reach service and utilise delegated discharge following discussion with the on-call Cardiologist when appropriate. Be first responder to patients within the Trust suspected to require the Primary Percutaneous Coronary Intervention (PPCI) service and activate the PPCI pathway after confirmation with Interventional Cardiologist. To contribute to the organisation of multidisciplinary, patient centred care. To be accountable for the planning and delivery of safe, effective and compassionate patient care in relation to dedicated pathways of care, working as an autonomous practitioner within Cardiology. To provide specialist clinical advice and support to patients and clinical staff within the Trust on the assessment, management and care of patients within Cardiology. To continuously improve clinical care standards in line with the clinical governance framework. To exercise accountability as set out in the NMC Code. To ensure effective communication internally and externally regarding patient care and resources. Provide education and training to clinical staff across the Trust. Provide expert specialist clinical advice and support to patients and clinical staff within the Trust on the management and care of patients within Cardiology including those with chest pain, undergoing coronary intervention or device implantation. To provide direct access for patients, either face to face or telephone consultation. To represent the service nationwide. To play a key role in ensuring that national and local standards/targets for Cardiology are met including those set out in the National Service Framework for Coronary Heart Disease (Department of Health 2000) and relevant National Institute of Clinical Excellence guidance. To manage/lead practitioner led clinics in relation to reviewing patients and play a key role in optimising complex treatment regimes, facilitating hospital discharge and preventing avoidable hospital admissions. To contribute towards audit activities across the service and undertake research relevant to the needs of the Cardiology department. To comply with the trusts vision, values and behavioural compact. Support peoples equality, diversity and rights. Promote best practice in health and safety issues. To undertake nurse led elective cardioversion list and also emergency cardioversions when required including taking informed consent for the procedure. Actively participate in post take ward rounds with medical team. To contribute to delivering a 24/7 service which includes rotation from day to night duty and possible cross site working. Support the Cardiology nursing team with the delivery of direct nursing care when available to do so. Support the cardiology nursing team across the unit to determine the correct course of action if there are any clinical concerns regarding a patient and support junior members of the team to escalate medical level of care as appropriate. Qualifications

Essential

Registered Nurse. Degree in nursing studies or relevant clinical degree (UK equivalent). Mentorship certificate / Practice Assessor training or demonstrable equivalent experience. Clinical skills accreditation*Consideration will be made for those who do not have clinical skills qualification but have appropriate Cardiology qualifications / experience and are willing to complete. Relevant Cardiology qualifications. Non-medical prescribing or willingness to complete.

Leadership and Management course. ALS.

Experience of care delivery. Experience of acute Cardiology. Experience of leading a team/supporting junior staff. Evidence of innovation in practice. Evidence of change management. Evidence of practice development. Experience of coordinating care for a caseload of patients.

Proficiency in: defibrillation, assisting with coronary angioplasty, temporary pacing wires, pericardial drains, IABP, CPAP and airvo.

Evidence of competency in relation to clinical practices appropriate to the post.
