An exciting opportunity has arisen foran experienced nurse to join the Heart Failure Specialist Nursing Team in SouthTyneside and Sunderland NHS.

You will need to have a sound knowledgeof cardiology nursing with a desire to develop skill and knowledge applicableto heart failure.

You will work autonomously and haveresponsibility for proactively managing a caseload of heart failure patients ina Primary and Secondary care setting.

We will offer you pathway ofprogression, training, and career structure and this post will develop to aBand 7 upon completion of developing practice in clinical skills module and independentNon-Medical Prescribing qualification.

Main duties of the job

We are part of the cardiothoracicdirectorate, and you will work within a team of heart failure specialist nursesand the wider cardiology team based at South Tyneside and Sunderland FoundationTrust.

Provide comprehensive, effective andspecialist nursing care and advice to patients, carers and health careprofessionals.

Assess, plan, develop and evaluateprogrammes of patient care in collaboration with the Heart Failure SpecialistNursing team.

Provide pharmacological management ofevidence-based therapies to reduce mortality and improve life expectancy inline with national policy.

It is essential you have a full UKdriving licence and access to a car.

A specialist nurse is defined as a qualified nurse whose primary role is to provide care, guidance and support which is based on an advanced level of knowledge to patients, carers and health professionals concerning a specific disease/condition.

The specialist nurse will:

Work as part of the multidisciplinary team working under the direction of line manager/Matron. Continue to develop advanced knowledge and skills within the specialist sphere of practice. Support with the planning, delivery and evaluation of care pathways for a designated group of patients under the direction of the line manager/Matron. Support the development of nursing practice within Cardiology through the use of evidence based practice and in line with agreed national standards, and contribute to the body of nursing knowledge. Work as an autonomous practitioner within their sphere of practice. Continuously improve clinical standards in line with clinical governance framework. Exercise accountability as set out in the NMC Code. Contribute to training and education within the specialist area. Provide specialist clinical advice and support to patients and clinical staff within the Trust on the management and care of patients within the specialist area. Provide direct access for patients, either face to face or telephone consultation Represent the service nationwide in the absence of the line manager/Matron. Play a key role in ensuring that national and local standards/targets for the specialist area are met. Manage and lead nurse led clinics in relation to reviewing patients and play a key role in optimising complex treatment regimes, facilitating hospital discharge and preventing avoidable hospital admissions. Support and undertake research relevant to the needs of the specialist practice. Comply with the Trusts vision, values and behavioural compact. To work closely with the Cardiology Team and support community cardiology services. Person Specification

Qualifications

Essential

Current 1st level NMC registration Degree in nursing studies or relevant clinical degree or evidence that currently working towards this (UK equivalent) Mentorship certificate/Practice Assessor training or demonstrable equivalent experience Clinical skills accreditation or willingness to complete

Desirable

Leadership and Management Course Basic knowledge of financial management Non-medical prescribing or willingness to complete
