An opportunity has arisen to join the Steps to Health team in Sunderland as an Exercise Practitioner. This is an exciting role working across the borough to support patients to become more active and reduce the impact of health conditions on the quality of patients lives.
Steps to Health are a dynamic, multi-disciplinary team who support and rehabilitate people in the community with a range of long term conditions which include: heart failure, cardiology, diabetes and respiratory. The Team is mainly based at Grindon Primary Care Centre and Springwell Health Centre but will work across the city and in community bases.
Aspects of the job include working closely with nurses, physiotherapists and other Exercise Practitioners in the team to undertake assessments of patients, complete gym inductions, physical activity goal setting, and one to one and group class session delivery. Exercise sessions and education of patients will be individualised and dependent on their health condition.
