Job Summary

This is an exciting opportunity for an experienced nurse to join the Heart Failure Specialist Nursing Team in South Tyneside and Sunderland NHS Foundation Trust as a Heart Failure Specialist Nurse. The successful candidate will have a sound knowledge of cardiology nursing and a desire to develop skills and knowledge applicable to heart failure.

Main Responsibilities
  • Provide comprehensive, effective, and specialist nursing care and advice to patients, carers, and healthcare professionals.
  • Assess, plan, develop, and evaluate programmes of patient care in collaboration with the Heart Failure Specialist Nursing team.
  • Provide pharmacological management of evidence-based therapies to reduce mortality and improve life expectancy in line with national policy.
  • Work autonomously and have responsibility for proactively managing a caseload of heart failure patients in a Primary and Secondary care setting.
  • Develop and lead nurse-led clinics in relation to reviewing patients and play a key role in optimising complex treatment regimes, facilitating hospital discharge, and preventing avoidable hospital admissions.
About the Role

The successful candidate will be part of the cardiothoracic directorate and work within a team of heart failure specialist nurses and the wider cardiology team based at South Tyneside and Sunderland Foundation Trust. The role will involve working closely with the Cardiology Team and supporting community cardiology services.

Person Specification
  • Current 1st level NMC registration.
  • Degree in nursing studies or relevant clinical degree or evidence that currently working towards this (UK equivalent).
  • Mentorship certificate/Practice Assessor training or demonstrable equivalent experience.
  • Clinical skills accreditation or willingness to complete.
Desirable Qualifications and Experience
  • Leadership and Management Course.
  • Basic knowledge of financial management.
  • Non-medical prescribing or willingness to complete.
  • Experience of coordinating care for a caseload of patients.
  • Experience in conducting research and critiquing clinical evidence.
  • Experience in implementing quality frameworks.

  • Understanding of contemporary nursing issues.
  • Evidence of continuous professional development.
  • Knowledge of how to facilitate learning.
  • Leadership skills.

  • Venepuncture, cannulation, and IV drug administration.
  • Proven interpersonal skills.
  • Practice assessor/practice assessor skills.
  • Evidence of competency in relation to nursing practices appropriate to the post.
  • Time management.
  • Ability to assess, deliver, evaluate, and benchmark quality care.
Physical Skills
  • Ability to assist patients to move as per Trust Moving and Handling Policy.
  • Carrying teaching equipment such as laptop, stationary, etc.
  • Manual dexterity and up to date working knowledge of equipment syringe drivers, infusion pumps, etc.
  • Full UK driving licence and access to a car is essential to this role.
