Job Title: Heart Failure Specialist Nurse

We are seeking an experienced and skilled Heart Failure Specialist Nurse to join our team at South Tyneside and Sunderland NHS Foundation Trust. As a key member of our Heart Failure Specialist Nursing Team, you will play a vital role in providing high-quality care to patients with heart failure.

About the Role

This is an exciting opportunity for a dedicated and compassionate nurse to work in a dynamic and supportive environment. As a Heart Failure Specialist Nurse, you will be responsible for:

  • Providing comprehensive and specialist nursing care to patients with heart failure
  • Assessing, planning, developing, and evaluating programs of patient care in collaboration with the Heart Failure Specialist Nursing team
  • Providing pharmacological management of evidence-based therapies to reduce mortality and improve life expectancy in line with national policy
  • Working autonomously and taking responsibility for proactively managing a caseload of heart failure patients in a Primary and Secondary care setting
Requirements

To be successful in this role, you will need:

  • Current 1st level NMC registration
  • Degree in nursing studies or relevant clinical degree or evidence that currently working towards this (UK equivalent)
  • Mentorship certificate/Practice Assessor training or demonstrable equivalent experience
  • Clinical skills accreditation or willingness to complete
