About the Role

We are seeking an experienced and skilled nurse to join our Heart Failure Specialist Nursing Team at South Tyneside and Sunderland NHS Foundation Trust. As a key member of our team, you will play a vital role in providing high-quality care to patients with heart failure.

Key Responsibilities
  • Provide comprehensive and specialist nursing care to patients with heart failure, including assessment, planning, and evaluation of care plans.
  • Work autonomously and collaboratively with the multidisciplinary team to deliver high-quality patient care.
  • Develop and implement evidence-based therapies to reduce mortality and improve life expectancy for patients with heart failure.
  • Provide pharmacological management of heart failure, including the use of medications and devices.
  • Collaborate with healthcare professionals to develop and implement care pathways for patients with heart failure.
  • Contribute to the development of nursing practice within Cardiology, including the use of evidence-based practice and national standards.
  • Represent the service at a national level, as required.
Requirements
  • Current 1st level NMC registration.
  • Degree in nursing studies or relevant clinical degree.
  • Mentorship certificate or Practice Assessor training.
  • Clinical skills accreditation or willingness to complete.
  • Leadership and Management Course (desirable).
  • Basic knowledge of financial management (desirable).
  • Non-medical prescribing or willingness to complete (desirable).
Desirable Qualities
  • Experience of care delivery.
  • Experience of the specialism, which would normally be gained through working at Band 5.
  • Evidence of leading a team.
  • Evidence of leading innovation in practice.
  • Evidence of leading change management.
  • Evidence of practice development.
