About the Role

We are seeking a highly skilled and motivated Advanced Nurse Practitioner to join our team at Monkwearmouth Health Centre in Sunderland. As a key member of our primary care team, you will play a vital role in delivering high-quality patient care and contributing to the development of our specialist services.

Key Responsibilities
  • Provide direct access to patients for minor illnesses and ailments, working autonomously and as part of a team to deliver effective care.
  • Conduct thorough assessments, make diagnoses, and develop care plans in collaboration with patients, carers, and other healthcare professionals.
  • Prescribe medications and treatments, ensuring safe and effective use of national prescribing standards.
  • Work closely with GPs, practice nurses, and other healthcare teams to ensure seamless care coordination and patient outcomes.
  • Contribute to the development and delivery of specialist primary care services, such as long-term disease management, and participate in shared learning across the practice and wider organization.
Person Specification
  • Registered General Nurse with an MSc or equivalent qualification.
  • Advanced Nurse Practitioner qualification or equivalent experience.
  • Proven ability to work autonomously and as part of a team, with excellent communication and interpersonal skills.
  • Knowledge of national standards, policies, and legislation that inform practice, including NICE guidelines and NSF.
  • Ability to manage patient risk effectively and safely, with a strong understanding of medico-legal aspects and equal opportunity issues.
