We are seeking an experienced and skilled nurse to join our Heart Failure Specialist Nursing Team at South Tyneside and Sunderland NHS Foundation Trust. As a key member of our team, you will play a vital role in providing high-quality care to patients with heart failure.
Key Responsibilities- Provide comprehensive and specialist nursing care to patients with heart failure, including assessment, planning, and evaluation of care pathways.
- Work collaboratively with the multidisciplinary team to deliver evidence-based therapies and improve patient outcomes.
- Develop and maintain advanced knowledge and skills in cardiology, including pharmacological management and non-medical prescribing.
- Contribute to the development of nursing practice within Cardiology, including the use of evidence-based practice and national standards.
- Exercise accountability and leadership skills, including the ability to manage and lead nurse-led clinics.
- Represent the service nationally and contribute to the development of national and local standards for heart failure care.
- Current 1st level NMC registration.
- Degree in nursing studies or relevant clinical degree.
- Mentorship certificate or practice assessor training.
- Clinical skills accreditation or willingness to complete.
- Leadership and management course or experience.
- Basic knowledge of financial management.
- Non-medical prescribing or willingness to complete.
- Leadership and management experience.
- Experience of coordinating care for a caseload of patients.
- Experience in conducting research and critiquing clinical evidence.
- Experience in implementing quality frameworks.
