About the Role

We are seeking an experienced and skilled nurse to join our Heart Failure Specialist Nursing Team at South Tyneside and Sunderland NHS Foundation Trust. As a Cardiovascular Care Specialist, you will play a vital role in providing comprehensive and effective care to patients with heart failure.

Key Responsibilities
  • Assess, plan, develop, and evaluate programs of patient care in collaboration with the Heart Failure Specialist Nursing team.
  • Provide pharmacological management of evidence-based therapies to reduce mortality and improve life expectancy in line with national policy.
  • Work autonomously and have responsibility for proactively managing a caseload of heart failure patients in a Primary and Secondary care setting.
  • Support the development of nursing practice within Cardiology through the use of evidence-based practice and in line with agreed national standards.
  • Contribute to training and education within the specialist area.
  • Provide specialist clinical advice and support to patients and clinical staff within the Trust on the management and care of patients within the specialist area.
Requirements
  • Current 1st level NMC registration.
  • Degree in nursing studies or relevant clinical degree or evidence that currently working towards this (UK equivalent).
  • Mentorship certificate/Practice Assessor training or demonstrable equivalent experience.
  • Clinical skills accreditation or willingness to complete.
Desirable Qualifications and Experience
  • Leadership and Management Course.
  • Basic knowledge of financial management.
  • Non-medical prescribing or willingness to complete.
  • Experience of coordinating care for a caseload of patients.
  • Experience in conducting research and critiquing clinical evidence.
  • Experience in implementing quality frameworks.
