Cardiovascular Rehabilitation Nurse Specialist

We are seeking an experienced and skilled Cardiovascular Rehabilitation Nurse to join our team at South Tyneside and Sunderland NHS Foundation Trust. As a specialist nurse, you will play a critical role in providing high-quality care and support to patients with heart failure.
Key Responsibilities- Provide comprehensive and effective nursing care to patients with heart failure, including assessment, planning, and evaluation of care plans.
- Work autonomously to manage a caseload of patients, providing proactive and specialist nursing care in both primary and secondary care settings.
- Develop and implement evidence-based therapies to reduce mortality and improve life expectancy for patients with heart failure.
- Collaborate with the Heart Failure Specialist Nursing team and wider cardiology team to provide seamless care and support to patients.
- Provide pharmacological management of evidence-based therapies, in line with national policy and guidelines.
- Contribute to the development of nursing practice within Cardiology, using evidence-based practice and national standards.
- Exercise accountability as set out in the NMC Code, ensuring high standards of care and professionalism.
- Contribute to training and education within the specialist area, sharing knowledge and expertise with colleagues.
- Manage and lead nurse-led clinics, reviewing patients and optimizing complex treatment regimes.
- Work closely with the Cardiology Team and support community cardiology services, ensuring seamless care and support for patients.
- Degree in nursing studies or relevant clinical degree, or evidence of working towards this qualification.
- Mentorship certificate or Practice Assessor training, or demonstrable equivalent experience.
- Basic knowledge of financial management.
- Non-medical prescribing or willingness to complete.
- Experience of care delivery, preferably in a specialist area.
- Experience of coordinating care for a caseload of patients.
- Experience in implementing quality frameworks and standards.
- Understanding of contemporary nursing issues and evidence of continuous professional development.
- Ability to assess, deliver, evaluate, and benchmark quality care.
- Counselling and appraisal skills.
- Full UK driving licence and access to a car, essential for this role.
