Job Summary

We are seeking an experienced nurse to join our Heart Failure Specialist Nursing Team in South Tyneside and Sunderland NHS Foundation Trust. As a Cardiovascular Rehabilitation Nurse Specialist, you will play a key role in providing comprehensive, effective, and specialist nursing care and advice to patients, carers, and healthcare professionals.

Main Responsibilities
  • Assess, plan, develop, and evaluate programmes of patient care in collaboration with the Heart Failure Specialist Nursing team.
  • Provide pharmacological management of evidence-based therapies to reduce mortality and improve life expectancy in line with national policy.
  • Support the development of nursing practice within Cardiology through the use of evidence-based practice and in line with agreed national standards.
  • Exercise accountability as set out in the NMC Code.
  • Contribute to training and education within the specialist area.
  • Provide direct access for patients, either face-to-face or via telephone consultation.
  • Play a key role in ensuring that national and local standards/targets for the specialist area are met.
  • Manage and lead nurse-led clinics in relation to reviewing patients and play a key role in optimising complex treatment regimes.
  • Work closely with the Cardiology Team and support community cardiology services.
Requirements
  • Degree in nursing studies or relevant clinical degree or evidence that currently working towards this (UK equivalent).
  • Mentorship certificate/Practice Assessor training or demonstrable equivalent experience.
  • Basic knowledge of financial management.
  • Non-medical prescribing or willingness to complete.
Desirable Qualities
  • Experience of care delivery.
  • Experience of the specialism, which would normally be gained through working at Band 5.
  • Experience of coordinating care for a caseload of patients.
  • Experience in implementing quality frameworks.
  • Understanding of contemporary nursing issues.
  • Evidence of continuous professional development.
  • Evidence of competency in relation to nursing practices appropriate to the post.
  • Ability to assess, deliver, evaluate, and benchmark quality care.
  • Counselling and appraisal skills.
Physical Skills
  • Ability to assist patients to move as per Trust Moving and Handling Policy.
  • Manual dexterity and up-to-date working knowledge of equipment e.g. syringe drivers, infusion pumps, etc.
  • Full UK driving licence and access to a car is essential to this role.
