Main duties of the job

The role involves taking calls from a variety of people including ward staff, care homes and families of patients in hospital. Ensuring that all Excel logs are up to date and correct and making entries onto the Mosaic system which is a directory used by Kirklees Council. The Admin Team support the Social Work Team to facilitate safe and appropriate discharge from the Mid Yorkshire Teaching NHS Trust.

Key Responsibilities

Responsible for tracking the progress andmonitoring of all Assessment notifications anddischarge notifications; printing off the system and ensuring accurate recordsare kept and responsible for passing onto the correct Social Worker in accordancewith the Care Act 2015. Reporting anydeviances to line management. To provide assistance upon request to thehospital social work team manager. Assist in the production of presentationmaterials using Microsoft software for the production of letters and casereports. The creation and subsequent production of themonthly Social Work Management report.Collect, collate and analyse complex information to produce monthlystatistics including data reports and budget clinics. The typing of highly sensitive case records,carer assessments, care management options, placement summary andcorrespondence using computerised word processing packages to support safedischarge from hospital within the priorities and timescales set by Kirklees Council. To attend a variety of highly confidential andsensitive meetings and arrange these meetings appropriately with a range ofmultidisciplinary staff groups from within the Social care setting and healthcare setting; ( safeguarding adult meetings, care planning meetings and avariety of other team meetings) to take detailed notes and subsequentpreparation of accurate minutes which can be used by other agencies such asWest Yorkshire Police, Her Majestys Coroner. To use Social Services computerised systems toinput patient information / assessment and care plan details in order tomaintain up-to-date accurate records and statistical information. This includes maintaining the integrity ofthe database by ensuring that documents are uploaded in a timely manner: - Scanning of Documents To create and upload documents onto CareDirector Kirklees Council systems. To input service packages onto Care Director KirkleesCouncil systems. To ensure that cases are closed appropriately bychecking and sorting information on the Care Director Kirklees Council system. To check that data on Care Director Kirklees Councilsystem is inputted by social workers accurately and complies with Kirklees Council,and Trust standards. In addition,responsible for data cleansing and data integrity of Care Director KirkleesCouncil system. Update Outlook calendar daily, bookappropriately, and edit as required. This function is utilised for bookingmeeting rooms within the health and social care setting. To provide an effective telephone service andinformation to the public, patients, and staff in other agencies to enablepeople to gain access to services which will identify / meet their needs. To speak to relatives over the telephonedaily. There may be occasions when thepost holder may have to speak to patients and their relatives in difficultcircumstances who may have terminal illnesses or request sensitiveinformation which may not be allowed to share. Previous word processing experience in office environment. Experience of dealing with people from varying backgrounds.

Data Protection, Caldicott, Health Care Act 2015 knowledge.

Qualifications

Essential

Advanced typing / word processing qualification equivalent to RSA III. NVQ Level 3 in relevant subject Business Administration or Equivalent experience working in social care/health setting.

Desirable

A recognised IT qualification equivalent to ECDL. Good spoken and written English.
