Social Work Support Administrative Officer

Key Responsibilities To ensure that the systems / databases are kept up to date in accordance with documented Kirklees Council and Dewsbury Hospital health setting policies and procedures. Updating and collating all information from the database including the production of a management report. Liaison with Health and Kirklees Council professionals to ensure the safe and appropriate discharge of patients. Responsible for tracking the progress and monitoring of all Assessment notifications and discharge notifications; printing off the system and ensuring accurate records are kept and responsible for passing onto the correct Social Worker in accordance with the Care Act 2015.
Reporting any deviances to line management. To provide assistance upon request to the hospital social work team manager. Assist in the production of presentation materials using Microsoft software for the production of letters and case reports. The creation and subsequent production of the monthly Social Work Management report.
Collect, collate and analyse complex information to produce monthly statistics including data reports and budget clinics. The typing of highly sensitive case records, carer assessments, care management options, placement summary and correspondence using computerised word processing packages to support safe discharge from hospital within the priorities and timescales set by Kirklees Council. To attend a variety of highly confidential and sensitive meetings and arrange these meetings appropriately with a range of multidisciplinary staff groups from within the Social care setting and health care setting; (i.e. safeguarding adult meetings, care planning meetings and a variety of other team meetings) to take detailed notes and subsequent preparation of accurate minutes which can be used by other agencies such as West Yorkshire Police, Her Majestys Coroner.
To use Social Services computerised systems to input patient information / assessment and care plan details in order to maintain up-to-date accurate records and statistical information. This includes maintaining the integrity of the database by ensuring that documents are uploaded in a timely manner: - Scanning of Documents To create and upload documents onto Care Director Kirklees Council systems. To input service packages onto Care Director Kirklees Council systems. To ensure that cases are closed appropriately by checking and sorting information on the Care Director Kirklees Council system.
To check that data on Care Director Kirklees Council system is inputted by social workers accurately and complies with Kirklees Council, and Trust standards. In addition, responsible for data cleansing and data integrity of Care Director Kirklees Council system. Update Outlook calendar daily, book appropriately, and edit as required. This function is utilised for booking meeting rooms within the health and social care setting.
To provide an effective telephone service and information to the public, patients, and staff in other agencies to enable people to gain access to services which will identify / meet their needs. To speak to relatives over the telephone daily. There may be occasions when the post holder may have to speak to patients and their relatives in difficult circumstances i.e. who may have terminal illnesses or request sensitive information which may not be allowed to share.
