They will support theClinical directors in coordinating all key activity including improving accessto services, providing advice and information and ensuring health and careplanning is timely, efficient and patient centered.
They will work closely andin partnership with the Social Prescribing Link Workers and support delivery ofthe comprehensive model of care to reflect local priorities, healthinequalities or population health management.

Ensure that patient records are accurately maintained.
Scanning of clinical correspondence into our clinical systems.
Ensure that clinical correspondence is received and processed inan accurate and timely manner through the Docman system.
Active document management, reviewing clinical correspondencebefore electronically work flowing to the GPs.
Ensure correspondence, reports, results etc.
are filed in records Screening and Recalls Ensure familiarity with all appointment systems including regularand incidental variations.
Book appointments and recalls ensuring sufficient information isrecorded to enable retrieval of the medical record.
Proactively contact patients to invite them for cancer screeningprogrammes, immunisations and long term condition reviews.
Administrative supportProvide administrative support to members of the primary healthcare team in the following areas ensuring appropriate practice records are keptup to date o Cytology (smears)o Results (x-ray, blood tests etc)o Seasonal and targeted Immunisation recalls o Private medical insurance reports Creation of patient searches (in EMIS, our clinical system) tosupport the coordination of project work across the hub.
Able to build relationships with ARRS staff, service users andallied professionals in member practices; to help identify patients who couldbe referred to other health care professionals within the PCN to help improvePCN efficiencies.
Provide coordination and navigation to patients across the PCN (PrimaryCare Network) Help people to manage their needs through answering queries,signposting to available services, and ensuring that people have good qualitywritten or verbal information to help them make choices about their care.
Support delivery of thecomprehensive model of care to reflect local priorities, health inequalities orpopulation health management.
Holistically bring together all of a persons identified careand support needs, and explore options to meet these using all availableservices and resources.
