We are seeking a skilled Acute Care Coordinator to join our team at Care New England Health System. As an Acute Care Coordinator, you will play a critical role in ensuring seamless transitions between hospital and home-based care, working closely with patients, families, and healthcare providers to deliver exceptional patient-centered care.
Job Summary:
The Acute Care Coordinator will be responsible for coordinating patient transitions, managing care plans, and communicating effectively with healthcare teams. This role requires strong organizational and communication skills, as well as the ability to work effectively in a fast-paced environment.
Responsibilities:
- Coordinate patient transitions between hospital and home-based care
- Manage care plans, including medication management and education
- Communicate effectively with healthcare teams, including physicians, nurses, and social workers
- Collaborate with other healthcare professionals to ensure smooth transitions
Requirements:
- Rhode Island Registered Nurse licensure required; Bachelor's Degree (min) in relevant discipline strongly preferred
- Minimum of one (1) year progressive clinical experience required
- Valid driver's license and automobile liability insurance required
- Ability to read and interpret medical records, reports, etc.
- Excellent oral and written communication skills required
Estimated Salary: $70,000 - $90,000 per year
