Patient Care Coordinator

As a Patient Care Coordinator, you will be responsible for managing a caseload of patients from admission to transition. This involves:
- Coordinating care with interdisciplinary teams, including doctors, nurses, and therapists.

A Bachelor's Degree in a relevant discipline is preferred, but equivalent education for RI licensure is also accepted. The estimated annual salary for this role is $100,000 - $160,000, based on industry standards and location.
