Job Title: Physiotherapist Care Manager

The Physiotherapist Care Manager is responsible for managing a caseload of patients from admission to transition into the community. This role coordinates interdisciplinary care, monitors treatment, and identifies opportunities for process improvement.

Responsibilities:
  • Manage patient caseloads in accordance with organization policies and regulations.
  • Coordinate interdisciplinary care teams to achieve positive therapeutic outcomes.
  • Monitor and adjust treatment plans to ensure effective patient care.
Requirements:
  • Equivalent education required for RI licensure.
  • Bachelor's Degree in relevant discipline preferred.
  • A minimum of one year progressive clinical experience required.
  • Valid CPR certification and RI Board license.
  • Estimated Salary: $65,000 - $80,000 per annum.
