Job Summary:

Care New England Health System seeks a highly skilled and dedicated Registered Nurse (Part Time) to join its team. As a Home Health Clinician, you will be responsible for managing a caseload of patients from admission to transition into the community, coordinating interdisciplinary care, and identifying opportunities for process improvement.

Responsibilities:

  • Manage a caseload of patients from admission to transition into the community.
  • Coordinate interdisciplinary care, including referrals when appropriate.
  • Monitor and adjust treatment to achieve positive therapeutic outcomes.
  • Identify opportunities for process improvement.

Requirements:

  • Equivalent education required for RI Registered Nurse licensure; Bachelor's Degree in a relevant discipline strongly preferred.
  • Valid RI Registered Nurse license required.
  • A minimum of one year progressive clinical experience required.
  • Valid CPR certification required.
  • Valid driver's license and automobile liability insurance are required.
  • Excellent communication and problem-solving skills required.
