Churches of Christ in Queensland offers a rewarding opportunity for an experienced Clinical Care Specialist to join our Home Care team. As a Registered Nurse, you will be responsible for delivering high-quality clinical care to clients residing in their own homes throughout Queensland.

This is a full-time permanent position offering a competitive salary range of $65,000 - $90,000 per annum, depending on experience and qualifications.

About the Role:

  • Deliver advanced clinical care to clients with complex needs, including chronic disease management, medication management, palliative care, and wound management;
  • Assess, plan, implement, and evaluate nursing care for home-based clients, ensuring that care plans are developed and reviewed based on individual client needs;
  • Monitor and ensure that client documentation is accurately reflective of care provisions and meets organisational requirements;
  • Provide timely and appropriate health education to clients and carers, as well as coordinate the collection of clinical indicator data;
  • Work collaboratively with allied health professionals and other clinical staff to deliver comprehensive care services.

Requirements:

  • Post-graduate experience as a Registered Nurse;
  • Strong understanding of primary health care principles, including consumer-directed frameworks;
  • Possession of First Aid and Cardiopulmonary Resuscitation (CPR) certificates;
  • QLD driver's license with access to a reliable and comprehensively insured vehicle (an allowance will be provided);
  • High level computer skills and excellent communication and interpersonal skills.
