Care Coordinator

As a Care Coordinator with The Care Bureau, you will be responsible for providing high-quality care to individuals in their homes. This role requires a strong focus on delivering excellent patient outcomes and promoting independence.
Key Responsibilities:
- Delivering personalized care plans to meet the unique needs of clients.
- Providing support with daily living tasks, such as bathing, dressing, and medication management.
- Collaborating with healthcare professionals to ensure seamless care transitions.
We offer a competitive salary of £20,000 - £25,000 per annum, depending on experience. You will also receive comprehensive training, including our standard carer training and advanced courses in Reablement, Risk Assessment, Peg Feeding, Wound Care, and Stoma Care.
