Primary Care Clinical Specialist

**Job Summary**
We are seeking a highly skilled Primary Care Clinical Specialist to join our team at Care New England Health System. As a clinical pharmacist, you will provide ambulatory pharmaceutical care services, working closely with our interdisciplinary care team to optimize medication regimens and promote high-quality patient care.
**Key Responsibilities**
- Function as the medication expert on the care team, developing and implementing strategies to improve patient outcomes.
- Provide medication and disease state management education to patients, promoting self-management and adherence to treatment plans.
- Collaborate with providers and patients to ensure high-quality, cost-effective care, documenting all encounters within the electronic health record.
- Contribute to the development and implementation of quality improvement initiatives, enhancing patient care and promoting best practices.
**Qualifications**
To succeed in this role, you will need a strong background in pharmacy, with a Doctor of Pharmacy (PhD) degree or a Bachelor's degree in Pharmacy with 3 years of experience. Current RI Pharmacist license is required, and PGY-1 Residency or Board of Pharmaceutical Specialties certification is preferred.
