Primary Care Advanced Practitioner

We are seeking a highly skilled Primary Care Advanced Practitioner to join our team at Rother House Medical Centre in Stratford upon Avon. As an integral part of our healthcare team, you will play a vital role in delivering high-quality patient care and contributing to the development of our practice.
Job OverviewThis is an exciting opportunity for an experienced healthcare professional to work in a dynamic general practice setting. Our practice serves 16,500 patients and we are committed to providing exceptional care within the resources available to us.
About the Role- You will assess, diagnose, and treat patients in practice premises or their own homes who require medical attention, referring to appropriate agencies as necessary.
- Proactively assess and manage frail patients within their own homes to develop advance care plans that support care in the community.
In this role, you will work autonomously, making professionally autonomous decisions, and collaborate with other healthcare professionals to deliver evidence-based care. You will also lead PCN clinical projects aligned with the PCN DES contract, focusing on health inequalities, Learning Disabilities, Long Term conditions, and Flu and Covid vaccination campaigns.
Responsibilities- Assess, diagnose, and implement programmes of evidenced-based care for primary care patients, undertaking re-evaluation and implementation of treatment plans as required.
- Hold clinics for patients with undifferentiated and undiagnosed problems, making assessments based on highly-developed knowledge and skills.
- Maintain effective time management and planning skills, adhering to clinical policy and procedure, and participating in clinical risk management.
A Postgraduate qualification within your clinical discipline, along with certification to prescribe, is essential for this role. We prefer candidates with prior experience in a Primary Care setting and the ability to make professionally autonomous decisions.
Requirements- Previous experience working in Primary Care.
- Competence in using Office and Outlook software.
- Effective time management and planning skills.
- Ability to follow clinical policy and procedure.
- Experience with clinical risk management.
- Experience of working in primary care or out-of-hours clinics.
The estimated salary for this position is £45,000-£60,000 per annum, depending on experience.
