About the Position:
Care New England Health System is seeking a skilled Care Coordination Navigator to join our team. The successful candidate will have experience navigating patients through complex healthcare systems and identifying resources to support their needs.
Responsibilities:
- Navigate patients through the healthcare system to access necessary resources and services.
- Identify and secure community resources to support patients with SDOH issues.
- Develop and implement care plans to address patient needs.
- Collaborate with primary care teams to promote patient-centered care.
- Monitor patient progress and adjust care plans as needed.
Requirements:
- High School diploma with relevant training and skills.
- Certification in Community Health Work or working toward certification within 12 months.
- Bachelor's degree in Social Work, Community/Public Health, or related field (preferred).
- Experience working with primary care providers and in healthcare settings.
Estimated Salary: $62,000 - $80,000 per year.
