We are seeking a highly skilled Nursing Care Coordinator to manage a caseload of patients from admission to transition.
The successful candidate will coordinate interdisciplinary care, monitor treatment plans, and identify opportunities for process improvement.
- Coordinate care with interdisciplinary teams to ensure seamless patient transitions.
- Monitor and adjust treatment plans in collaboration with healthcare professionals.
- Identify opportunities for process improvement to enhance patient outcomes.
Requirements:
- Bachelor's Degree in a relevant discipline preferred.
- Equivalent education for RI licensure required.
