Job Summary: As a key member of the Care New England Health System team, the Medical Care Coordinator plays a vital role in ensuring seamless transitions for patients from admission to community discharge. Reporting to the Team/Program Manager, this position involves coordinating interdisciplinary care plans, monitoring treatment outcomes, and identifying opportunities for process improvement. The ideal candidate will possess strong communication skills, a keen eye for detail, and the ability to work effectively within a dynamic healthcare environment.

A competitive salary of approximately $64,000 - $72,000 per annum, commensurate with experience, reflects the importance of this role in delivering high-quality patient care.

Responsibilities:
