About the Role

Key Responsibilities:

  • Collaborate with primary care teams to promote patient-centered care.


Requirements:
  • High School diploma or equivalent required.
  • Certification in Community Health Work or working towards certification within 12 months.
  • Associates or Bachelor's degree in Social Work, Community/Public Health, or related field preferred.
