Quality Improvement Coordinator

Job summary To support the practice inquality improvement programmes with a focus on improving patient care.Use clinical systems to extract dataused to identify, recall, prioritise and contact patients requiring healthchecks or reviews.
This requires a degree of confidence, computer literacy,accuracy, to be methodical and logical building of searches (full training begiven).

Job description Job responsibilities Coordinate review appointmentsfor patients with chronic diseases and/or medications that require monitoringand contact where necessaryContact patients with learningdisability or severe mental illness prior to their annual health check andsupport them to attendUse the NHS Cervical ScreeningManagement System (CSMS) to check invites sent appropriately and follow upnon-responders etcFacilitate registering of childregistrations, ensuring children are followed up for immunisation appointments(especially those from overseas) and liaise with Child Health Manage vaccination recalls forchildhood immunisation programmes as well as recalls for seasonal flu, covid,pneumonia and other NHS vaccines as per national guidance Liaise with patients and otherhealthcare providers to coordinate care Perform advanced recordmanagement for patients undergoing adoption or gender transitioning Assist with medication management(including questionnaires, prescriptions) and coordinate review appointmentsManage the carers register,providing support and information as required as well as identifying potentialcarers on the practice list Support the IT department throughinforming patients of test results, actioning clinical post and assisting withIT problems Assist with QOF (Quality OutcomeFramework) ensuring targets metAssist with CQRS (CalculatingQuality Reporting Service) ensuring correct funding received for servicesundertaken through Direct Enhanced Services (DES) and Locally CommissionedServices (LCS)Manage Research related admin,coordinate projects, contact patients accordingly, ensure payments for researchreceived and attend any required training for complianceAttend/present at meetings asrequired relevant to job role Assist with ad-hoc cover forother admin depts if necessary Ensure records are updated andcoded appropriately Supporting in the planning andmonitoring of projects in accordance with PCN (Primary Care Network)requirementsPlease note:This job description is not exhaustive.
