About the Opportunity

We are looking for an enthusiastic and motivated individual to join our Homefirst Team, which works within the integrated community team in West Cornwall.

As a key member of our team, you will be responsible for assessing and providing care plans for adults within the catchment area. You will also work closely with other healthcare professionals, fostering strong relationships and effective communication.

Job Requirements
  • Hold a degree or diploma in Occupational Therapy, Physiotherapy, or Nursing, and be HCPC/NMC registered.
  • Have significant post-registration clinical experience in a similar role.
  • Demonstrate excellent communication and interpersonal skills.
Compensation and Benefits
