About the Role

We are seeking a highly skilled Mental Health Support Worker to join our team in Cornwall. As a key member of our community, you will be working closely with vulnerable adults who require support and guidance.

Job Description
  • Provide one-to-one support to customers living in the community, ensuring their safety and wellbeing.
  • Work collaboratively with specialist providers to deliver tailored support plans, helping customers achieve greater independence and access local services.
Key Responsibilities
  1. Develop and maintain relationships with referral partners, including local Mental Health Teams and Adult Social Care in Cornwall Council.
  2. Conduct regular risk assessments to ensure the service remains suitable for customers.

The ideal candidate will have excellent communication skills, be able to work under pressure, and possess accurate record-keeping abilities.
