The Job Overview
- Support customers in achieving their goals, promoting independence, and accessing local services.
- Collaborate with specialist providers to deliver tailored support that addresses each customer's unique needs.
- Develop and maintain relationships with referral partners, including local Mental Health Teams and Adult Social Care in Cornwall Council.
- Conduct regular assessments to ensure the service remains suitable for customers.
- Manage caseload effectively and complete regular risk assessments.
Essential Skills and Qualifications
- Understanding of support planning and support needs.
- Experience in risk assessment and risk management.
- Strong communication and relationship-building skills.
- Accurate record keeping and computer proficiency.
- Confidence in lone working and ability to work under pressure.
- Familiarity with welfare and housing benefits.
- Safeguarding expertise.
Working Conditions
This is a full-time, 2-month contract with the possibility of extension. You will be based in the Cornwall area and receive an essential car user allowance as driving between customers is a significant part of the role.
