About the Opportunity

As a Community Care Assistant in Cornwall, you will have the chance to make a real difference in the lives of vulnerable individuals. The Job Summary

  • Provide personalized support to customers living in the community with severe and enduring mental health diagnoses.
  • Collaborate with specialist providers to deliver support that promotes independence, access to local services, and community involvement.
  • Establish strong relationships with referral partners, including local Mental Health Teams and Adult Social Care in Cornwall Council.
  • Conduct ongoing assessments to ensure the service meets the needs of customers.
  • Maintain accurate records and complete regular risk assessments.

Required Skills and Qualifications

  • Knowledge of support planning and support needs.
  • Experience in risk assessment and risk management.
  • Effective communication and relationship-building skills.
  • Accurate record keeping and computer proficiency.
  • Comfortable working independently and meeting deadlines.
  • Familiarity with welfare and housing benefits.
  • Safeguarding expertise.

Working Arrangements

This is a field-based role, and an essential car user allowance will be provided. You will be required to drive between customers, making this role ideal for those with a valid driver's license and a reliable vehicle.
