Senior Nurse Practitioner

Cornwall Partnership NHS Foundation Trust is seeking an experienced Senior Nurse Practitioner to join our medical team at Camborne & Redruth Community Hospital. The successful candidate will work alongside GPs and ACPs in assessing and managing patients presenting to our Same Day Emergency Care unit (SDEC) and who have been admitted on Cornwall's Virtual Ward.
SDEC provides an alternative to the acute hospital for patients who can be safely managed outside of the emergency department with the aim of home on the same day. The follow-up to this is provided by our Virtual Ward. Patients are kept at home with regular nursing contact and medical oversight 7 days a week. This is a Consultant-led service with opportunities to join ward rounds and MDTs for learning and service provision.
We also host a Urgent Treatment Centre and Community Assessment and Treatment Unit (CATU). The post holder may rotate through these services depending on skill and service requirement. The UTC sees a range of medical problems covering all age groups, self-presenting to the unit. The CATU provides short-stay, ward-based medical and nursing care, with therapy assessments.
Main Responsibilities:
- Conduct history-taking, examination, requesting investigations and creating an appropriate safe medical management plan for patients presenting to the services above.
