This is an exciting opportunity to join our experienced team as a Dementia Adviser. As a Dementia Adviser, you will play a vital role in providing personalised support and information to people diagnosed with dementia and their carers, helping them to maintain their independence.

**Key Responsibilities**

  • Improve people's sense of well-being and control of their lives.
  • Assist with the identification of needs, providing information and access to relevant services.
  • Accurately record your work on our database information system.
  • Visit clients in their own homes, so be able to travel independently across the borough.
  • Promote our service and build relationships with a range of health and social care professionals.

**Requirements**

  • You will need to be self-motivated and resilient, an empathic team player who pays attention to detail.
  • We particularly welcome applications from people who are representative of the communities we serve in the borough.
  • You will have determination to support people to maintain their independence, improve their sense of well-being and put them in more control of their lives.
  • You will have compassionate understanding of the needs of people affected by dementia.
  • You will have ability to work inclusively with all people affected by dementia, along with an understanding of the need for confidentiality.

The estimated salary for this role is £32,000 - £40,000 per annum, depending on experience. We also offer a range of benefits, including pension scheme, life insurance, and 25 days' annual leave.
